Job summary

Tri-County Community Action Agency in Rhode Island is seeking a Case Manager for Adult Protective Services. You will provide case management for adults age 60+ referred by the RI Office of Healthy Aging, both in the field and in the office.

Travel statewide, perform home visits, assess needs, and coordinate services to support independence and safety. The position requires an Associate degree or higher in a related field, plus at least two years of relevant experience.

Qualifications

• Associates or higher degree in gerontology, human services, rehabilitation, social work, psychology, or related field.
• Two years of experience providing case management or related services to adults 60+.

Responsibilities

• Provide case management services for adults 60+ referred by RI OHA APS.
• Conduct in-person home visits and develop an individualized plan of care.
• Coordinate wraparound services with healthcare providers, families, and community partners.
• Maintain case notes in the OHA APS database and comply with policies.
