Job summary

The Rescue Mission of Utica in Utica, NY is seeking an Adult Protective Case Manager to oversee services under the Rep Payee Program and assist clients with essential needs, safety and support planning.

Responsibilities include monthly visits, referrals processing, reporting, and collaboration with OCDFCS to monitor client progress. A valid NY driver’s license and 3+ years in human services are required.

Qualifications

• Bachelor’s degree in Human Services, Social Work, Sociology, Psychology, or equivalent.
• Minimum 3 years of experience in Human Services with a public or private agency.
• Experience with one-to-one interaction with clients to identify needs and goals for service planning.
• Proficient in Microsoft Word, Excel and Outlook.
• Valid NYS driver's license and own transportation.

Responsibilities

• Conduct face-to-face and home visits to assess client safety and determine needs at least monthly.
• Screen program referrals from OCDFCS and meet with staff and client within 3 days of referral.
• Determine client daily living needs and assist in meeting those needs.
• Meet with OCDFCS to discuss client status monthly or as requested.
• Provide monthly status reports including financial and personal details.
• Maintain accurate client files and progress notes for permanent records.
