Job summary

Tri-County Community Action Agency seeks a dedicated Case Manager for Adult Protective Services in North Providence/Johnston, RI. The role focuses on delivering comprehensive case management for adults 60+ in collaboration with RI OHA and APS units.

You will assess needs, develop individualized care plans, and coordinate services to support safe, dignified aging at home. The position requires in-person visits, strong communication, and ability to travel across the state, with adherence to

Qualifications

• Associates or higher degree in gerontology, Human Services, Rehabilitation, Social Work, Psychology, Social Services, Behavioral Health Science, or related field.

Responsibilities

• Provide case management services for adults aged 60 and older in Rhode Island as referred by RI OHA and the APS unit.
• Conduct in-person, unannounced home visits for self-neglect and post-abuse cases within priority timeframes.
• Develop and implement individualized care plans and coordinate services addressing all needs identified in assessments.
• Provide care coordination, advocacy, and wraparound services in collaboration with families and healthcare providers.
• Travel statewide to client homes as required and maintain timely documentation in the OHA APS database.

Education
