Job summary

General Responsibility

Overall responsibility for providing case management services for persons in R.I. age 60 and older as referred by The Rhode Island Office of Healthy Aging (RI OHA), Adult Protective Services(APS) Unit. The Office of Healthy Aging Adult Protective Services Unit is responsible for investigating reported concerns of elder abuse, neglect, exploitation, or self-neglect in R.I. for persons age 60 and older. OHA contracts with Tri-County Community Action Agency to provide case management services.

Specific Responsibilities

Support older adults living safely, with dignity and independence within the community by responding to reports to older adults that may be self-neglecting, need early intervention, or OHA investigator referred. Responsible for short term case management support and services to a caseload 40-60 consumers. Conduct in person, unannounced home visits for all self-neglect and post abuse cases, within priority timeframes as prescribed by OHA. Complete an APS assessment of the clients needs, with agreement and input of the client. Develop and implement an individualized plan of care and recommended services that address all pertinent areas from the comprehensive needs assessment. Provide care coordination, advocacy and related case management activities interested cases.

Work collaboratively with client, client’s family, medical providers, ancillary health care providers, elder law advocates, first responders, police, housing and building code enforcement, residential service coordinators and community service providers to provide wraparound services to clients

Provide follow up to the client following the initial assessment for a minimum of threemonths to ensure continuity of care. Assist elders in gaining access to community health, housing, transportation, food and support services. Coordinate care with OHA to ensure that client’s needs are addressed in an integrated manner. Utilize OHA APS electronic data base through timely completion of required documents and case record notes. Attend meetings and trainings at or required by RI OHA as necessary. Comply with all OHA and Tri-County Community Action Agency policies and procedures and with all current state and federal laws and regulations related to confidential client information

Work may include occasional exposure to unpleasant and unsanitary living conditions. Interactions with elders who may have poor hygiene, residing in unsafe living conditions, have mental health issues, substance abuse issues, and homelessness.

Qualifications

Possess an Associates or higher degree, from an accredited college or university, in gerontology, Human Services, Rehabilitation, Social Work, Psychology, Social Services, Behavioral Health Science, or related field.

Two years of relevant experience (e.g. providing case management or other type of assistance) working with the target population, elders over the age of 60, consumers who are self- neglecting, possess physical and or cognitive impairments, mental health and or substance abuse issues, and facing eviction or homeless.)

Special Qualifications

Possess strong organizational, verbal and written skills as well as strong computer skills with Microsoft Office products. Working knowledge of the provision of health care in a variety of settings. Familiarity with pertinent elder abuse dynamics and the legal system. Knowledge of community resources. Ability to communicate and work collaboratively with elders, families and community resources. Ability to work with various cultures. Knowledge of social, health, and mental health interventions. Ability to analyze situations, problem solve, and make immediate decisions in crisis situations. Ability to create and sustain trusting relationships with clients and families and practice active listening skills. Be available to travel throughout the state. Maintain an active driver’s license, maintain automobile insurance and have access to reliable transportation. Pass a state and national criminal background check. Ability to work in an office environment and the field. Travel to client’s homes across the catchment area. Work may include occasional exposure to unpleasant and unsanitary living conditions.
