Job summary

Jewish Association For Services For The Aged is seeking an Elder Abuse Prevention Social Worker / Case Manager in New York City. The role involves assessing needs, creating safety plans, and delivering counseling across home, office, and community settings.

You will coordinate with protective services, legal advocates, and community resources to establish long‑term stability, while traveling across NYC for client home visits and maintaining detailed electronic records.

Qualifications

• Master’s Degree in Social Work (MSW) or equivalent in Gerontology, Psychology, Human Services, or related field.
• Clinical experience in individual and group counseling.
• Ability to travel NYC via public transportation for home visits.

Responsibilities

• Assess and plan safety for seniors reporting abuse across in-home, in-office, and community settings.
• Provide counseling using evidence-based modalities (e.g., Motivational Interviewing, trauma-informed care).
• Navigate benefits (Medicaid, Medicare, SNAP, Cash Assistance, Home Care, Meals on Wheels, transportation).
• Coordinate referrals to legal services and community resources.
• Facilitate group counseling to reduce isolation and build peer networks.
• Maintain audit-ready electronic case notes and assessments.
