Job summary

The Snoqualmie Indian Tribe is seeking an Elder Care Program Coordinator to support Tribal Elders through home visits, needs assessments, and connection to medical, social, and community resources. You will document, assist with care planning, and coordinate services within the Tribal community.

Level I duties include conducting visits, evaluating care plans, arranging transportation, and maintaining records; Level II expands with policy support and enrollment assistance using Tribal Assister

Qualifications

• Education and experience in elder care or related human services.
• Proficiency with Microsoft Office and record-keeping.
• Ability to work with diverse populations and maintain confidentiality.

Responsibilities

• Conduct home visits and welfare checks to assess Elders’ needs.
• Coordinate transportation and connect Elders to community resources.
• Maintain accurate client records and documentation for care planning.
• Support Elder program activities and trainings.
