Job summary

The Snoqualmie Indian Tribe is seeking an Elder Care Program Coordinator to support Tribal Elders through home visits, care planning, and connections to medical, social, and Tribal resources. The role builds trust, assesses safety and daily living needs, coordinates services, and documents progress.

Position is full-time hourly in Snoqualmie, WA with hybrid work arrangements. Benefits include medical/dental/vision, 401k, paid holidays, and paid time off.

Qualifications

• Associate degree in human services or related field required or equivalent experience.
• Bachelor’s degree in social work, public health, human services, or related field preferred.
• Experience in case management and working with seniors preferred.

Responsibilities

• Conduct home visits and welfare checks for Tribal Elders to assess well-being and needs.
• Coordinate services and care plans with Elders, families, and providers.
• Maintain accurate client records and documentation.
• Assist Elders with navigating community resources and transportation coordination.
