Job summary

The Sault Ste. Marie Tribe of Chippewa Indians seeks an Elder Outreach Worker to build relations with tribal elders and families, primarily visiting homes, clinics, and community settings. You will assess needs, coordinate programs, and connect elders with services to maintain quality of life and aging in place.

Regularly report to the Elder Services RN and promote elder involvement in aging programs. The role includes coordinating home care services, developing resource directories, and

Qualifications

• Education: High School diploma or GED required.
• Experience: 1+ years in social service delivery with elderly/family population preferred.
• Certification/License: background check required; valid driver’s license; insurable by tribal insurance; follow drug-free policy.
• Knowledge/Abilities: knowledge of tribal culture; strong communication; ability to plan, prioritize and manage workload; confidentiality.

Responsibilities

• Network with community organizations to offer elder services.
• Conduct psychosocial assessments and referrals for tribal elders and families.
• Provide case management and coordinate services for aging in place.
• Maintain documentation and report to Elder Services RN.
• Coordinate meals-on-wheels, transportation, and other elder program services.
• Attend trainings and promote elder program participation.
