Job summary

Southwest Youth & Family Services is seeking a dedicated Case Manager to support high school students reengaging in education. Qualifications

• BA preferred in social work, education, counseling, or related field; 2+ years direct multi-service experience.
• Knowledge of juvenile justice court systems and navigation of resources for youth.

Responsibilities

• Coordinate with schools, employers, colleges and agencies to keep youth engaged in programs.
• Recruit students and conduct comprehensive needs assessments for reengagement.
• Develop Individual Student Learning Plans to help students reach education goals.
• Provide career guidance, referrals to social and health services, and job search support.
• Advocate for youth and collaborate with community partners to remove access barriers.
• Maintain student records and meet monthly reporting deadlines.
• Be available outside work hours for student needs.
