Job summary

Hospice of the Midwest in Edina, MN seeks a dedicated Community Liaison to promote hospice services to healthcare professionals, families and community partners from a central Edina location. This full-time role emphasizes building durable relationships, coordinating outreach, and driving awareness at events and clinics.

You will manage outreach with CRM tools, stay updated on hospice care best practices, and support smooth patient referrals with the care team.

Qualifications

• Bachelor's degree in a related field and prior experience in healthcare sales or hospice outreach required.
• Advanced degree or certification in healthcare marketing, and experience working with CRM systems and referral networks.
• Minimum of 2 years of healthcare marketing experience.
• Possess a current and valid state driver’s license.
• Provide proof of auto insurance.
• Demonstrate strong interpersonal and communication skills.

Responsibilities

• Visit hospitals, clinics, and senior care facilities to promote our hospice services.
• Build strong, ongoing relationships with doctors, nurses, and referral partners.
• Attend community events to raise awareness and represent our brand.
• Track and manage outreach activity in our system (CRM).
• Use market insights to plan and improve outreach efforts.
• Educate families and professionals on hospice benefits and how we can help.
• Work with our care team to ensure smooth patient referrals and quality care.
• Stay current on hospice care through training and self-learning.
• Complete reports, documentation, and other admin tasks on time.
• Follow all healthcare rules and regulations.
