Qualifications

• Master's Degree in Social Work (MSW) from an accredited college or university.
• Current Maryland Licensed Master Social Worker (LMSW) license.
• Minimum of two years of experience in emergency department social work or related fields.
• Experience conducting psychosocial assessments and discharge planning in acute care.

Responsibilities

• Conduct comprehensive psychosocial assessments for emergency department patients and families.
• Provide crisis intervention, emotional support, de-escalation, and counseling in a fast-paced ED.
• Perform safety assessments and interventions for behavioral health crises, homelessness, and substance use.
• Develop and implement patient-centered discharge plans to support safe transitions of care.
• Coordinate referrals to community resources, housing, behavioral health services, and social services.
• Collaborate with physicians, nurses, case managers, psychiatric teams, and community partners.
• Participate in rounds, treatment planning, and case reviews to improve outcomes.
• Advocate for patients and families and promote access to healthcare and social supports.
• Maintain EMR documentation in line with standards and participate in quality improvement.
• Provide mentorship to social work students and interns as assigned.
