Qualifications

Responsibilities

• Medication administration, transcription, and documentation following Hospital policies.
• Develop, communicate, and update individualized plan of care for each patient.
• Assess and reassess patient needs and document outcomes per nursing standards.
• Facilitate discharge/transfer planning and address barriers with the care team.
• Maintain patient safety through accurate identification, hand hygiene, and regulatory compliance.
