Qualifications

• Employ a nursing process to deliver individualized, outcome-oriented care.
• Coordinate care with the health care team and family to meet patient needs.
• Administer, transcribe, and record medications per policy.

Responsibilities

• Develop, evaluate and update an individualized plan of care and document outcomes.
• Initiate admission assessment within 8 hours of admission and identify teaching needs.
• Continuously assess and document patient care following unit standards and policies.
• Document all patient care per nursing policy, policy, and disease processes.
• Assess pain and patient response to interventions, pharmacological and non-pharmacological.
• Facilitate discharge/transfer planning and communicate barriers to the team.
