Responsibilities

• Coordinate patient care in emergency setting and support timely, appropriate decisions.
• Collaborate with ED providers and care teams to assess patient needs upon presentation.
• Identify levels of care and facilitate admissions, transfers, or safe discharges.
• Coordinate care across inpatient, observation, and post‑acute settings.
• Support patient flow and throughput initiatives and discharge planning assessments.
• Apply medical necessity and utilization criteria and ensure regulatory compliance.
• Educate patients and families on care plans and next steps.
