Community Medical Centers in California seeks an RN Case Manager for the Emergency Department (ED) to coordinate care across the continuum and ensure efficient patient throughput to the appropriate level of care.

You will collaborate with patients, Case Management, physicians and community agencies to optimize utilization; support inpatient or observation status decisions with the Utilization Review Coordinator.

Qualifications

• Bachelor's Degree in Nursing required.
• 1 year of acute care hospital experience required.
• 2 years of acute Case Management experience and/or 1 year of Emergency Department experience preferred.
• 4 years of acute care hospital experience may be substituted in lieu of Bachelor's Degree and experience requirement.
• RN license to practice within California required.
• BLS required.
• ACM and CCM certifications preferred.

Responsibilities

• Coordinate care across the continuum to facilitate patient throughput.
• Collaborate with patients, Case Management team, and physicians to manage cases.
• Assist in identifying patients appropriate for inpatient or observation admission status with Utilization Review.
