Job summary

Trinity Health is seeking an RN Case Manager in the Emergency Department to coordinate patient care, assess patients on arrival, and facilitate safe, timely decisions. The role focuses on reducing avoidable admissions and readmissions by linking patients to community resources and supporting discharge planning from entry to exit.

Collaborate with physicians and interdisciplinary teams to improve throughput, align with CMS guidelines, and promote high-quality, patient-centered care across the ED

Qualifications

• Current GA RN license.
• Case Management Certification preferred.
• BSN recommended.

Responsibilities

• Uses admission and continued stay criteria to ensure appropriate setting.
• Collaborates with physicians for level of care decisions.
• Completes timely initial and concurrent reviews.
• Implements discharge/treatment plans to prevent unnecessary inpatient stays.
• Maintains HIPAA compliance and CMS Participation.
