Job summary

St. Mary’s Health Care System is seeking an RN Case Manager for the Emergency Department to coordinate patient care, optimize utilization of services, and support efficient patient flow.

You will work closely with physicians, nursing staff, and interdisciplinary teams to assess patients at presentation and facilitate timely, appropriate care decisions. This role emphasizes reducing avoidable admissions/readmissions by identifying alternate levels of care, linking patients with community

Qualifications

• GA RN license (required) with active status; Case management certification preferred.
• Education: Must be a graduate of an accredited nursing program; BSN recommended.
• Experience: Minimum 2 years in hospital Nursing; 1 year in case management/utilization or discharge planning preferred.

Responsibilities

• Uses approved admission criteria to ensure appropriate setting and timely plan of care.
• Collaborates with admitting/attending physicians for care level and status decisions.
• Completes biopsychosocial assessments to determine post-acute needs and safe discharge plans.
• Coordinates referrals to home health, hospice, SNFs, DME and other services.
• Maintains HIPAA compliance and CMS participation requirements; documents accurately.
