Job summary

Trinity Health in Georgia seeks a dedicated RN Case Manager for the ED to coordinate patient care, promote timely and appropriate utilization, and help improve patient flow.

You will work with physicians, nurses, and interdisciplinary teams to assess patients at presentation, support discharge planning, identify alternative levels of care, and connect patients with community resources.

Qualifications

• GA RN license required; BLS required.
• BSN recommended for nursing graduates.
• Case Management Certification preferred; minimum two years hospital nursing experience.

Responsibilities

• Utilizes CMS criteria to determine appropriate level of care.
• Collaborates with physicians and nursing staff on care decisions.
• Completes initial and concurrent reviews for admission/continuation.
• Assesses post-acute needs and facilitates safe discharge planning.
• Coordinates referrals to home health, hospice, SNFs, and other services.
• Maintains HIPAA compliance and documentation of care plans.
