Job summary

Northeast Georgia Health System in Georgia seeks an experienced RN to provide comprehensive assessment, planning, and coordination of patient care across the care continuum. This role emphasizes discharge planning, length of stay management, and collaboration with physicians, patient families, and a multidisciplinary team to ensure safe, cost-effective outcomes.

Prerequisites include Georgia RN licensure and an associate degree; 3–5 years of direct patient care or case management experience

Qualifications

• Licensed to practice as an RN in Georgia.
• Associates Degree; graduate of an accredited school of nursing.
• 3–5 years of direct patient care and/or case management experience.

Responsibilities

• Monitor patients and manage length of stay with efficient use of resources.
• Communicate with physician, patient/family, and other disciplines about stay and discharge progress.
• Coordinate and facilitate discharge planning for post-acute needs; ensure timely transitions.
• Collaborate with Utilization Review Nurse daily to align status and level of care.
• Lead discharge planning efforts and act as liaison to families and the care team.
• Support a customer service oriented environment within the care setting.
