Job summary

Trinity Health is seeking an RN Case Manager to coordinate patient care within the Emergency Department, working with physicians and nursing staff to ensure safe and timely care decisions. The role emphasizes reducing avoidable admissions and facilitating discharge planning from entry.

The ideal candidate has an RN license, at least 2 years in hospital nursing, and preferred case management certification. BSN is recommended and proficiency with CMS guidelines is essential to support population

Qualifications

• RN license valid in the applicable state; Case Management Certification preferred.
• Education: Graduate of an accredited school of nursing; BSN recommended.
• Experience: Minimum 2 years’ nursing in a hospital setting; 1 year in case management, utilization management, or discharge planning preferred.

Responsibilities

• Coordinate patient care activities in the ED to ensure appropriate utilization and timely care decisions.
• Collaborate with physicians, nursing staff, and interdisciplinary teams.
• Identify alternative levels of care and support discharge planning from entry.
• Maintain CMS/HIPAA compliance and documentation throughout care.
• Refer cases to home health, hospice, SNFs, or other services as needed.
