Baystate Health is seeking an experienced Crisis Clinician to join the Emergency Department team in Springfield, MA. This full-time position supports Baystate Medical Center and will require travel to related EDs in Westfield and Palmer, MA.

Minimum qualifications include a Master’s degree and LICSW eligibility, with preferred experience in behavioral health settings. Compensation notes reflect variable base salaries by grade across the system, with typical hours and state licensing

Qualifications

• Master's degree in Social Work required or similar.
• LICSW eligibility required or preferred per MA licensing rules.
• Experience in behavioral health setting preferred.

Responsibilities

• Obtains and maintains release of information forms and gathers collateral information from family, providers, and community resources.
• Conducts psychosocial evaluations including history, mental status, and risk assessment through interviews and chart reviews.
• Monitors and assists in preparing legal documentation (CVs, 3-Day Notices, Section 12s, Section 7/8 forms).
• Enters data and reviews charts for QA, outcomes tracking, and progress.
• Completes discharge planning addressing follow-up, step-down options, transportation, and treatment needs.

SCHEDULE:

Full-time, 40 hours/week

Must be able to travel to support Baystate Noble Hospital ED (Westfield, MA) and Baystate Wing Hospital ED (Palmer, MA)

LOCATION:

Baystate Medical Center- Springfield, MA

Baystate Noble Hospital- Westfield, MA

JOB SPECIFIC DUTIES/RESPONSIBILITIES

Obtains and maintains release of information forms, and gathers collateral information from outside resources including family, friends, providers, and other community resources.

Completes a psychosocial evaluation including current and past history, assessment of mental status, and risk assessment through patient interviews, collateral contacts, and record review.

Monitors and assists in the completion of legal documentation (CV's, 3-Day Notices, Section12's, & Section 7/8 forms).

Enters data and reviews charts for quality assurance, program statistics, tracking of patient progress, and/or tracking of clinical outcomes as assigned.

Completes a discharge plan which addresses follow-up recommendations, step-down / placement options, transportation issues, or other treatment needs.

Other duties and responsibilities as required.

MINIMUM QUALIFICATIONS:

Master's degree required.

LICSW or license eligible required.

Experience in behavioral health setting required.
