Job summary

Memorial Healthcare System is seeking an Entry Point Case Manager in Pembroke Pines, FL, focusing on utilization management and safe discharge planning for ED and other entry points in need of care. You will collaborate with the healthcare team to address patient needs and optimize care transitions.

The role requires 3 years of RN experience, with CCM certification preferred. Evening shift duties include coordinating resources, documenting plans, and supporting transitions to appropriate levels

Qualifications

• Three (3) years’ RN experience.
• Experience with medical necessity criteria, discharge planning in an acute care setting or ED setting.
• Certification of Case Management (CCM) preferred.

Responsibilities

• Identify and coordinate discharge planning for ED and entry-point patients to ensure safe transition.
• Collaborate with ED staff to determine admission status and optimize patient care paths.
• Track high ED utilizers and connect patients with community resources and appropriate services.
• Provide utilization management guidance and education to the care team to support appropriate care levels.
• Maintain accurate documentation and liaise with the Care Coordination Center for referrals and transfers.
