Job summary

Memorial Healthcare System in Hollywood, FL is seeking an RN Case Manager to coordinate care from admission through discharge. You will partner with interdisciplinary teams to develop comprehensive case management plans and ensure safe transitions of care.

Key requirements include an accredited nursing program, Florida RN license, and BLS certification. Minimum 1 year hospital case management or 5 years healthcare experience. Compensation based on experience.

Responsibilities

Consults with nursing staff and multidisciplinary team to evaluate patient status and care appropriateness.
Participates in discharge planning including coordinating patient transfers and community resources.
Provides discharge education and resource referrals to patients.
Monitors patient and family satisfaction.
Responds to questions and complaints from patients, family members, and payors.
May perform chart review to identify issues affecting service delivery, outcomes, compliance or reimbursement.
May collect and assess data to identify quality trends or risks to patients and staff.
Collaborates with multidisciplinary team and committees regarding quality outcomes.
