Qualifications

• Minimum one year of experience in a hospital-based Case Management practice or five years in Healthcare.
• Graduate of an accredited Registered/Professional Nursing Program.
• Basic Life Support (BLS) Healthcare Provider required upon hire.

Responsibilities

• Identifies and addresses patients with frequent ED use for non-urgent care or patients with high hospital admission rates.
• Provides support for other entry points as indicated, including responding to status requests in the PACU and supporting ED transfers and direct admits.
• Screens all ED patients with potential for admission identifying payer source.
• Develops relationships with community health resources to improve outpatient access to healthcare for ED population.
• Tracks clinic patients and frequency of ED use for non-acute care.
• Offers options for future non-acute care for clinic patients.
• Tracks barriers to clinic access.
• Supports medical record documentation.
• Performs utilization management focusing on appropriate admission status via criteria-based review and resource education.
• Assigns admitting status (observation vs admit).
• Collaborates with attending/ED staff for appropriate designation.
• Works with multi-visit patients to refer to ED Social Work and community resources.
• Provides discharge planning for ED patients with available services or placement options.
