Job summary

Montage Health is seeking an ED Clinical Care Coordinator to serve as the gatekeeper of the Emergency Department, guiding admission decisions, bed placement, and patient progression to support efficient flow.

You will collaborate with physicians, nurses, and interdisciplinary staff to anticipate admissions, coordinate dispositions, and ensure timely discharge planning and appropriate utilization of hospital resources.

Qualifications

• Bachelor’s degree in nursing is preferred.
• State RN license in California is required.
• BLS certification is required.

Responsibilities

• Serve as ED care coordination hub liaison for admissions, disposition, and bed placement.
• Identify barriers to admission and escalate to ensure timely patient progression.
• Collaborate with physicians and nurses to plan discharge and alternative care pathways.
• Coordinate interdepartmental referrals and utilization management.
• Educate staff on patient status, bed placement, and payer requirements.
