Job summary

LCMC Health is seeking a Care Manager who serves as the hub of patient case management and care planning. You will ensure quality through care coordination, collaborate with the care team, and implement plans and transition strategies to achieve optimal outcomes and resource use.

Schedule features 12-hour shifts with 8 am–8 pm hours and weekend rotations, requiring Louisiana nursing licensure and 2 years of nursing experience in care management or ED.

Qualifications

• Active nursing license to practice in Louisiana.
• 2 years of professional nursing experience in Care Management or ED.
• Experience coordinating across care teams and with patients and families.

Responsibilities

• Develop individualized care plans with prioritized goals aligned to patient and caregiver preferences.
• Make outbound calls to assess patient health status.
• Identify gaps or barriers in treatment plans.
• Coordinate care and refer to outside sources as needed.
• Ensure safe discharge planning with necessary services and medication reconciliation.
• Triage symptom-based calls per approved protocols.
• Enroll patients and manage alerts for home monitoring programs.
• Provide approved health information and guidance to callers.
• Document all workflows in the EPIC system for tracking and quality assessment.
• Educate patients on self-management of wellness and disease.
• Discuss clinical questions with care team and physicians to meet quality and cost objectives.
• Demonstrate energy, enthusiasm, and commitment to quality work.
