Job summary

LCMC Health is seeking a Care Manager to coordinate patient care, develop individualized plans, and support transitions across the continuum of care in Louisiana. You will collaborate with care teams to optimize outcomes, length of stay, and resource use.

Role requires an active Louisiana nursing license, 2 years of Care Management or ED nursing experience, and strong communication with patients and families. Schedule includes 12-hour weekend shifts and 8-hour weekday shifts.

Qualifications

• Current nursing license to practice in Louisiana.
• 2 years of professional nursing experience in Care Management or ED.

Responsibilities

• Develop individualized care plans with patient and caregiver goals.
• Make outbound calls to assess patients’ health status.
• Coordinate care and arrange referrals to external resources.
• Ensure safe discharge planning and medication reconciliation.
• Triage symptom calls per approved protocols.
• Educate patients on self-management and disease processes.
• Document workflows in EPIC for tracking and quality assessment.
