Job summary

Boston Medical Center seeks a Multi Visit Patient Project Clinical Manager to lead clinical services for a targeted care management program for high-utilization patients in the Emergency Department. The role requires collaboration with ED leadership, Population Health, Psychiatry, and addiction services, and will supervise social work students and MPH interns.

Qualifications

MSW required.
Active LICSW preferred; LCSW consideration with plan to licensure.
Minimum five years in social services, healthcare, or public health.
Experience in management/supervision; homelessness services valued.

Responsibilities

Engage high-utilization ED patients with care coordination and crisis planning.
Supervise and coach students and trainees; mentor staff.
Foster ED and clinical partner relationships; participate in huddles and bedside engagement.
Provide harm-reduction oriented care planning and transitions of care.
Document thoroughly in EMR and ensure data quality and billing compliance.
Collaborate with internal/external partners and community services.
Assist leadership with program analysis, administrative tasks, and strategy.
