Job summary

The MVPP (Multi Visit Patient Program) Clinical Manager in Boston oversees clinical services within BMC's Population Health Care Management, leading a team of social workers and students to improve care for high-utilization patients in the emergency department.

Collaborating with ED leadership, psychiatry, and community partners, the role emphasizes harm-reduction, care coordination, and data-driven program analysis while ensuring compliance with MassHealth requirements and internal standards.

Qualifications

• MSW with LICSW preferred or LCSW with relevant experience and desire to work towards LICSW licensure.
• Minimum five years of experience in social services, healthcare, or public health.
• At least one year of management/supervisory experience preferred.
• Prior experience in emergency services setting preferred.
• Prior experience providing case management to high-risk and complex populations preferred.

Responsibilities

• Address unmet clinical and social needs of super utilizers of the ED through direct patient engagement, care coordination, crisis planning, and resource connection.
• Supervise, coach, mentor students supporting the program including clinical supervision, training, coaching, and managing relationship with host institution.
• Build relationships with ED staff and other departments to meet the needs of a complex patient population.
• Utilize patient-centered and harm reduction approaches; support safety through care planning and transitions of care; make clinical decisions when needed.
• Complete thorough documentation in the EMR and comply with data entry, data integrity, and MassHealth billing requirements.
• Engage with internal and external partners across the health system and with city/state agencies and community shelters.
• Support program needs including data entry, program analysis, and strategic contributions.
