Qualifications

• Bachelor's degree in Social Work or related field.
• Minimum of two years of case management or workforce development experience.
• Experience serving individuals/families from diverse backgrounds.
• Strong organizational, time management, and documentation skills.
• Valid driver's license and reliable transportation preferred.

Responsibilities

• Conduct participant intake, assessments, and eligibility determinations.
• Develop individualized service plans with goals, strengths, barriers, and steps toward stability.
• Provide ongoing case management, coaching, and accountability.
• Build trusting relationships and maintain consistent participant engagement.
• Conduct regular follow-ups via in-person, phone, or virtual meetings.
• Monitor progress and adjust service plans as goals change.
• Apply a strengths-based approach to identify opportunities and milestones.
