Job summary

Universal Healthcare IPA, Inc. is seeking an Enhanced Care Management (ECM) Case Manager LVN in Fresno, CA. This full-time, non-exempt position requires in-person care coordination for members with complex health and social needs.

The LVN will operate under the Case Management Manager, conducting assessments, developing care plans, coordinating services, and engaging with the Interdisciplinary Care Team (ICT) to ensure continuity of care and successful outcomes.

Qualifications

• High School diploma or GED required.
• Current California LVN license required.
• Minimum of two years in a healthcare setting; managed care or IPA experience preferred.

Responsibilities

• Manage a caseload of ECM members.
• Develop and implement individualized care plans.
• Coordinate with interdisciplinary team and community resources.
• Conduct in-person care coordination and assessments.
• Facilitate transitions and discharge planning.
• Maintain accurate documentation and compliance.
