Job summary

Arrae Health Primary Care seeks a dedicated Community Health Worker to support the Enhanced Care Management (ECM) program for IEHP members. You will bridge the gap between providers and the community, helping patients navigate care, access resources, and improve health outcomes.

You will deliver personalized health education, care coordination, and support for patients with complex needs, addressing social determinants of health and connecting them to local resources to promote long-term health
