Job summary

Koinonia Family Services is seeking a part-time ECM Care Coordinator in Concord, CA. The role focuses on assessments, care plans, and service coordination for youth and families within Enhanced Care Management.

Responsibilities include client engagement, home visits, referrals, and progress monitoring, with an emphasis on bilingual communication and culturally competent care. Flexible shifts and remote work between sessions are offered.

Qualifications

• Minimum 2 years of case management (or related) experience.
• Proficient bilingual or multilingual abilities are a plus.
• Valid driver’s license, reliable personal vehicle and auto insurance.

Responsibilities

• Conduct assessments of individuals' needs and formulate individualized care plans.
• Enthusiastically educate and enroll clients about services, and motivate them to engage in services.
• Meet with individuals in their homes or agreed locations to facilitate services.
• Oversee and ensure seamless coordination of services (physical, mental, dental, social).
• Act as liaison between individuals and healthcare providers and social services.
• Refer individuals to appropriate services and resources as needed.
• Maintain regular contact to review care plans and monitor progress.
