Job summary

Koinonia Family Services seeks a part-time ECM Care Coordinator to support youth and families in the East Bay. You will assess needs, develop care plans, coordinate services, and educate clients to improve health and wellbeing. Bilingual ability is valued as we serve a diverse community.

The role offers flexible afternoons/early evenings shifts, remote work between in-person client sessions, and mileage reimbursement. Prior ECM or case management experience is beneficial.

Qualifications

• Two years of related work experience in social work, case management, community engagement or customer service.
• High school diploma or GED required.
• Bilingual or multilingual proficiency is a plus.

Responsibilities

• Conduct assessments of individuals' needs and formulate individualized care plans.
• Educate and enroll clients about services and motivate engagement.
• Meet clients in homes or community locations to facilitate services.
• Oversee and coordinate services including physical, mental, dental care and social services.
• Act as liaison between clients and healthcare providers; and social services.
• Refer clients to appropriate community resources as needed; monitor progress and outcomes.
• Document services according to ECM program standards; maintain confidentiality.
