Job summary

Public Health Management Corporation seeks a Resource & Referral Specialist for ELRC 18 to promote the Early Learning Resource Center as a key access point for families in Philadelphia. You will engage with families, providers, and community partners to connect individuals with needed services and benefits.

You will support the Family Services team, conduct outreach, and ensure timely, accurate documentation while navigating regional resources and eligibility processes across agencies.

Qualifications

• High school diploma or GED required; associate degree preferred.
• 1+ years in community services, planning, or related field.
• Ability to connect families with available resources and provide navigation support.

Responsibilities

• Provide timely responses to client outreach and referrals.
• Share information about OCDEL programs in community settings.
• Maintain accurate records using designated databases and applications.
• Meet with family clients to understand goals and care needs.
• Assist families in accessing external services and navigating intake steps.
• Develop relationships with ELRC team and community providers.
• Complete program requirements timely and ensure compliance with regulations.
• Participate in ongoing professional development related to ELRC policies.
