Job summary

Care Lync is seeking a Supports Coordinator (Early Intervention Services) to provide ongoing service coordination for children birth to age 6 and their families. You will help families access needed services, understand rights, and receive coordinated, high-quality supports that promote child development and family capacity.

Qualifications

• Bachelor's degree in one of the listed fields or related area.
• Minimum 1 year of experience working with infants, toddlers, or children with disabilities.

Responsibilities

• Coordinate and monitor service delivery as primary contact for families.
• Assist families in accessing needed services and scheduling appointments.
• Facilitate IFSP/FSP development, evaluations, and reviews.
• Educate families on rights and safeguards, promoting empowerment.
• Monitor services for IFSP alignment and coordinate across service systems.
• Maintain accurate records and documentation for compliance.
• Collaborate with families, providers, and agencies on planning and communication.

Skills

Experience with infants/toddlers

Education

Bachelor's degree in Social Work, Psychology, Sociology

Bachelor's degree in Early Childhood Education or Special Education
