Job summary

Center for Human Services (CHS) in Missouri seeks an Early Intervention Service Coordinator to support the SPOE team by coordinating services to families of infants and toddlers in the home, aligning with CHS mission and state requirements.

Responsibilities include leading IFSP processes, collecting data, coordinating with family resources, and traveling within the service area to ensure timely service delivery and documentation.

Qualifications

• Bachelor’s Degree in a listed field with 1 year of relevant experience.
• Or a BA/BS with 3 years of relevant experience.
• Valid driver’s license is required.
• Proof of personal vehicle insurance required.
• Background screenings and fingerprinting required.

Responsibilities

• Serve as IFSP team leader and coordinate intake and planning for First Steps.
• Collect family insurance, income, and household data for fee determination and tracking.
• Monitor service delivery and assist with referrals to community resources.
• Participate in IFSP meetings, EIT activities, and document attendance.
• Maintain accurate electronic and paper records per DESE contract; travel to family homes.
• Support the SPOE contract requirements and timelines as directed.
