Job summary

Thrive Alliance in Indiana is seeking a Service Coordinator to help families with a child aged birth to 3 with a disability access a comprehensive, community-based system of services in their home and local environment. You will coordinate IFSP, advocate for families, and connect them with early intervention services under Part C of IDEA, conducting in-home assessments and leading cross-agency teams.

Travel is required; a Bachelor's degree and state credentials are needed; strong communication

Qualifications

• Bachelor's degree in a human services-related field required.
• Strong verbal and written communication skills.
• Ability to travel with reliable transportation and a valid driver's license.
• Knowledge of resource/referral processes and state regulations.

Responsibilities

• Coordinate IFSP and lead teams including family, providers, and services.
• Conduct in-home assessments and advocate for families.
• Ensure ongoing eligibility and access to services under First Steps and IDEA Part C.
• Manage documentation and deadlines, working with minimal supervision.
