Job summary

Blue River Services, Inc. is seeking a Full-time First Steps Service Coordinator to provide service coordination for eligible infants and toddlers and their families in Indiana.

You will coordinate evaluations, develop IFSPs, ensure compliance with state and federal regulations, maintain accurate documentation, and support families in accessing services and community resources. The role requires travel within assigned areas, scheduling flexibility, and collaboration with a multidisciplinary team

Qualifications

• Bachelor’s degree required.
• Minimum of one year experience working with children and families.
• Knowledge of First Steps eligibility criteria and IFSP development.
• Strong organizational, documentation, and data-entry skills.
• Effective written and verbal communication skills.

Responsibilities

• Respond to referrals within 2 business days and initiate intake procedures.
• Complete data entry and maintain electronic records in EI Hub.
• Conduct intake, obtain informed consent, and explain safeguards and family rights.
• Collect required financial and insurance documentation for Cost Participation.
• Coordinate and ensure completion of evaluations and eligibility within 45 days.
• Facilitate development, review, and implementation of IFSPs with families and providers.
• Coordinate IFSP meetings, annual reviews, and transition conferences.
• Monitor and coordinate service delivery across providers and funding sources (including Medicaid and CSHCS).
• Assist families in accessing appropriate agencies and community resources.
• Maintain detailed and timely documentation, including monthly clinical logs and reports.
• Support families in transition planning to preschool or other services prior to age three.
• Obtain necessary medical documentation from healthcare providers.
• Provide services in home and community settings based on family needs.
• Maintain cooperative relationships with families, agencies, and community partners.
• Submit monthly reports and required documentation by deadlines.
