Qualifications

• Bachelor’s degree required plus minimum two years’ experience with young children birth through 5 years.

Responsibilities

• Coordinate and actively participate in public awareness and child find activities.
• Determine the nature of each referral and the appropriate follow-up contacts.
• Provide intake into the early intervention system, including obtaining information and releases.
• Inform families of rights and safeguards and advocacy services.
• With family consent, meet in home or another location to identify priorities and needs.
• Secure medical information/history and arrange additional evaluations as needed.
• Ensure eligibility for early intervention services is determined.
• Prepare families to participate in all aspects of the IFSP process.
• Coordinate and participate in development of the IFSP with family and team.
• Assist families in identifying resources and options.
• Maintain procedural safeguards, confidentiality, and timely service delivery.
• Maintain records and data; liaise with DEIC, PIPCs, and interagency groups.
