Amplified Therapy, Inc. in Deming, NM is seeking a Family Service Coordinator (FSC) to serve as the single point of contact for coordinating, scheduling, and monitoring early intervention services for families with children from birth to age three.

The FSC works across agencies, explains IFSP processes, helps families access services, participates in meetings, and ensures timely delivery while maintaining documentation and compliance with NM FIT program standards.

Qualifications

• A bachelor’s degree in social work or a related field is required.
• A second bachelor’s degree plus two years of relevant experience is acceptable.
• Training for family service coordination is required within one year of hire.

Responsibilities

• Act as the single point of contact for coordinating, facilitating and monitoring delivery of EI services.
• Coordinate services across agency lines to ensure timely provision of supports.
• Explain to families the early intervention process and procedural safeguards.
• Gather information from families regarding concerns, priorities and resources.
• Coordinate evaluations, assessments and eligibility determinations for IFSP.
• Schedule, organize and participate in IFSP meetings and coordination with providers.
• Coordinate funding sources for services listed on the IFSP.
• Maintain timely documentation and participate in quality management activities.

Education

Bachelor’s degree in social work, counseling, psychology, nursing, special education, or any other closely related field

Bachelor’s degree in another field plus two years of direct experience in serving individuals with disabilities and/or families
