SGA Youth in Chicago operates an Early Head Start Home-based program serving children 0-3 and pregnant parents. The Health & Mental Health Specialist monitors health screenings, individual health plans, and follow-up care for participating families.

The role centers on 90-minute weekly home visits, group activities, and referrals to services to support healthy child development. Bilingual Spanish/English is preferred; a valid driver's license is required.

Competenze

Associate degree in Early Childhood Education and 12 ECE credits or a Home Visitor CDA.
Gateways ECE certification completed or in process.
Two years of experience in Early Head Start preferred; bilingual Spanish/English preferred.

Mansioni

Provide 90-minute home visits weekly for 10 children and/or pregnant women.
Complete at least 46 home visits per child during the program year.
Plan and implement developmentally appropriate home-based activities with families.
Conduct developmental and social-emotional screenings (ASQ-3, ASQ-SE).
Enter observations into COR and support Family Partnership Plans.
Refer families to needed services and coordinate with ERSEA.
Maintain a full caseload and participate in trainings and meetings.
Report suspected child abuse or neglect.
