Job summary

SGA's Early Head Start Home-based program in Chicago seeks a Health & Mental Health Specialist to monitor health screenings, develop individual health plans, and ensure follow-up care for children aged 0-3 and pregnant parents.

The role emphasizes coordinating vision/hearing screenings, ASQ developmental assessments, and linking families to needed services while building strong caregiver partnerships and community collaboration.

Qualifications

• Associate’s degree in Early Childhood Education is required.
• At least 12 ECE credits and/or a Home Visitor CDA.
• Gateways ECE certification is required or preferred.

Responsibilities

• Provide and maintain 90-minute home visits for 10 children and/or pregnant women weekly.
• Conduct at least 46 home visits per program year.
• Plan developmentally appropriate activities with families.
• Complete weekly observations and enter into COR.
• Support health, nutrition, and development screenings per schedule.
• Refer families to needed services and follow up on goals.
