Minooka Community High School District 111 seeks an Early Childhood Assessment Facilitator for a one-year position. The role focuses on facilitating assessments, managing IEP processes, and ensuring smooth transitions for preschool children from early intervention services. Ideal candidates will possess a Professional Educators License and ECSE Endorsement, showing strong organizational and collaboration skills.
Maintain regular communication with Local Interagency Council (LCI) service coordinators to schedule and support Early Intervention (EI) transition meetings.
Participate in virtual meetings with EI families and service coordinators to complete referral forms and determine the need for school-based evaluations.
Provide tours of buildings and classrooms, along with explanations of a range of services available to preschool students.
Serve as the primary point of contact for scheduling team members for evaluations and meetings, in collaboration with assigned staff.
Facilitate completion of the initial Early Childhood Outcomes form as part of the child’s transition file.
Attend quarterly LIC meetings.
Work in collaboration with the special education team to prepare and draft domain paperwork.
Contact families to schedule monthly play-based assessments, eligibility meetings, and Individualized Education Program (IEP) meetings.
Facilitate or assist with play-based assessments for children approaching their third birthday.
Draft IEP goals and objectives in preparation for eligibility and IEP meetings.
Attend and participate in eligibility and IEP meetings alongside EI teams, assessment staff, and families.
Coordinate monthly early childhood screenings and play-based assessments.
Ensure families receive and complete the required documentation prior to screenings.
Review screening referrals and prepare for domain meetings with evaluation teams and families. Schedule evaluations for students referred through preschool screenings.
Conduct classroom observations of preschool students as requested.
Participate in planning meetings to support the development of preschool class lists and placements.
Manage documentation for grant compliance.
Performs such other, and not specifically enumerated, duties as may be requested by building principal, district administrators, or as established by the Board.

COMPENSATION
180-Day School Year, +5 per diem days as approved by the principal
Salary and School Year to be established by the Board
EVALUATION
The performance of the job will be evaluated in accordance with the Teacher Evaluation Plan.
