Job summary

The Cleveland Clinic Bath Health and Wellness Center seeks a Professional Health Coordinator/EAP Counselor to support caregivers and healthcare professionals through assessment, case management, and counseling. The role involves collaboration with hospital leadership and cross-organization teams to ensure high-quality employee assistance services.

On-site four days at Bath Health and Wellness and one day at Mercy, with a Monday–Friday core schedule and occasional evening coverage.

Qualifications

• Master’s degree in Behavioral Sciences.
• LISW or LPCC license.
• Minimum five (5) years of direct clinical services experience.
• Case management and assessment experience.

Responsibilities

• Provide assessment, coordination, case management and monitoring of participants in the EAP program.
• Consult hospital leadership on performance and personal-issue related problems.
• Provide short-term counseling and supportive services to participants.
• Promote the program and build relationships with hospital personnel; conduct presentations.
• Staff critical cases with Caring for Caregivers leadership and relevant committees.
• Complete all documentation per professional and departmental standards.
