Job summary

Heritage Manor Care is seeking a Physical Therapy Assistant to join our post-acute team in a residential care setting. The PTA will provide therapy services under the supervision of a PT, coordinate with physicians and family, and contribute to patient progress notes and discharge summaries.

Qualified candidates will have an Associate of Arts in Physical Therapy Assistant, CPR certification, and CA licensure/certification as required.

Qualifications

• Associate degree in Physical Therapy Assistant.
• State licensure/certification as required.
• CPR certification required.

Responsibilities

• Provide therapy services per the patient Plan of Care under the supervision of a PT.
• Consult with PT regarding changes in treatment or condition in a timely manner.
• Complete daily notes and assist PT with progress notes, discharge summaries or monthly reports.
• Submit billing, payroll, and expense documentation in a timely manner.
• Communicate with patients, families, referring physicians and team members.
• Attend meetings and training to stay current with clinical skills.
