Job summary

Apploi is seeking an energetic Physical Therapy Assistant to participate in therapy for residents in long-term and rehabilitation settings. You will contribute to resident-centered treatment plans and educate team members to ensure high-quality care.

This role requires hands-on care, reliability, and experience supervising staff. You will assist with discharge planning, including home care referrals and DME coordination, as part of an interdisciplinary care approach.

Qualifications

• 1–3 years experience as a PTA is required or preferred.
• Experience in Long Term Care settings is desirable.
• Reliable transportation is required.
• Able to deliver hands-on care to residents.
• Experience supervising, directing, training and coaching staff.
• Understanding of restorative or rehabilitation programs needed.
• Proficient with computer tasks and documentation.

Responsibilities

• Participate in the treatment plan and therapy sessions for residents.
• Contribute to resident-centered treatment plan design.

Job description

We are seeking an energetic, organized and self-motivated individual to make a difference in the therapy we provide our residents. The Physical Therapy Assistant will be responsible for the participating in the treatment, and resident centered treatment plan design for our long-term, geriatric and short term rehabilitation residents. You will be a part of our interdisciplinary approach to skilled care delivery and act as educator to other team members in order to enhance the care delivered from all aspects of patient care. You will be responsible for the communication of restorative nursing needs at the time of discharge. Your input is essential to the communication of discharge needs in terms of home care referrals, DME ordering and family training.

The Ideal Candidate Will Have:

• 1-3 years experience as a PTA, preferably in a Long Term Care setting;
• Reliable transportation;
• The ability to be a hands on care provider;
• Experience in supervising, directing, training and coaching employees;
• Understanding of restorative or rehabilitation programs; and
• Computer efficiency.
