Job summary

The Park in Plano, TX is seeking a dedicated Physical Therapist (PT) to join our in-house team. You will support residents’ mobility and quality of life through person-centered care.

Responsibilities include assessing patients, developing and implementing individualized plans, administering therapies, documenting care, and collaborating with the rehab team to ensure high-quality outcomes.

Qualifications

• Active and valid Texas Physical Therapist license.
• SNF or long-term care experience preferred.
• Knowledge of current physical therapy treatment practices.

Responsibilities

• Consult with patients to assess their physical condition and functional limitations.
• Evaluate and interpret patient assessments and test results.
• Develop individualized treatment plans using a variety of therapy techniques.
• Create personalized rehabilitation programs tailored to each patient’s goals.
• Administer medically prescribed physical therapy treatments to relieve pain, restore function, and promote healing.
• Ensure all treatments are delivered in accordance with the established plan of care.
• Monitor patient response and modify approaches in collaboration with the interdisciplinary team.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with interdisciplinary team members to ensure high-quality patient care.
• Complete documentation thoroughly and timely in compliance with policies and regulations.
• Attend required meetings as designated by the Director of Rehab.
