Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our team in SNF settings. You will evaluate and treat patients following an individualized plan of care, and you will educate patients about their injury, progress and probable outcomes.

Responsibilities include supervising Physical Therapist Assistants, re-evaluating patient progress, and maintaining timely documentation such as evaluations, re-evaluations, discharges and daily progress notes.

Qualifications

• Evaluating new patients and developing plans of care.
• Treating patients and educating them on injury, progress and outcomes.
• Supervising Physical Therapist Assistants and documenting progress notes.

• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
