Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join our SNF team. You will evaluate and treat patients following an individualized plan of care and collaborate with facility staff to deliver high-quality services.

As a Physical Therapist, you will supervise Physical Therapist Assistants, re-evaluate progress, adjust goals, and ensure timely documentation of evaluations, re-evaluations, discharges and daily progress notes.

Qualifications

• Licensed Physical Therapist in good standing.
• Ability to develop individualized plans of care.

Responsibilities

• Providing excellent care for patients.
• Collaborate with facility staff to ensure excellent care for patients.
• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Treating patients, addressing concerns and educating them on progress and outcomes.
• Supervising Physical Therapist Assistants.
• Re-evaluating patient progress, revising plans of care and resetting goals.
• Completing accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.
